What Does Medicare Cost in 2026?

In 2026, the standard Part B premium is $202.90 per month, and the annual deductible is $283. After you meet the deductible, Medicare typically pays 80% of the Medicare-approved amount for covered services, while you pay the remaining 20%.

For most beneficiaries, total monthly Medicare costs run $230–$270 for Parts B and D combined. But this figure understates costs for two groups: high-income beneficiaries who pay IRMAA surcharges, and beneficiaries without Medigap coverage who face significant out-of-pocket costs from the 20% coinsurance on all Part B services.

IRMAA 2026 — The Surcharge Most Beneficiaries Don't See Coming

IRMAA (Income-Related Monthly Adjustment Amount) is the most important Medicare cost that higher-income retirees consistently underestimate. For 2026, the IRMAA income brackets and surcharges increased approximately 3–9%. The surcharge applies to beneficiaries with income exceeding $109,000 for single filers or $218,000 for joint filers.

IRMAA works as a cliff. If your 2024 income is $109,001 as a single filer, you do not pay a small fraction more — you jump to the full next tier and owe $284.10 per month for Part B instead of $202.90. One dollar of additional income can trigger an $81.20 monthly increase.

The two-year lookback trap: Your 2026 IRMAA is based on your 2024 income. A one-time event in 2024 — selling a home, a large Roth conversion, an inheritance — can trigger IRMAA in 2026 even if your 2025 and 2026 income are well below the threshold. You can appeal IRMAA using SSA Form SSA-44 if your income dropped significantly after the determination year.

Original Medicare vs Medicare Advantage — Cost Comparison

Medicare Advantage (Part C) is an alternative to Original Medicare delivered through private insurers. It bundles Parts A, B, and usually D into one plan. Many Advantage plans have $0 premiums — but that does not mean $0 cost.

Frequently Asked Questions — Medicare Costs 2026

How much does Medicare cost per month in 2026?
The standard Medicare Part B premium in 2026 is $202.90 per month. Most people pay no Part A premium if they worked 40+ quarters (10 years). Part D averages $30–$60/month depending on plan. Total monthly cost for a typical beneficiary without IRMAA runs $230–$270 for Parts B and D combined. Higher-income beneficiaries pay up to $689.90/month for Part B due to IRMAA surcharges.
What is IRMAA and who pays it in 2026?
IRMAA is a surcharge added to Medicare Part B and Part D premiums for higher-income beneficiaries. In 2026, it applies when your 2024 MAGI exceeds $109,000 (single) or $218,000 (joint). The surcharge works as a cliff — exceeding the threshold by $1 triggers the full tier surcharge. Part B surcharges range from $81.20 to $487.00/month above the standard premium.
What is the Part A deductible in 2026?
The Medicare Part A hospital deductible in 2026 is $1,736 per benefit period — up $60 from 2025. This applies per hospitalization benefit period, not annually. Days 61–90 cost $433/day in coinsurance. After 90 days, lifetime reserve days cost $866/day. Medigap plans cover most or all of these costs.
Can I appeal an IRMAA surcharge?
Yes. You can appeal IRMAA if your income dropped significantly due to a life-changing event — retirement, divorce, death of a spouse, loss of income, or reduction in work hours. File SSA Form SSA-44 with Social Security. If your appeal is approved, Medicare recalculates your premium using more recent income data.
Does Medicare cover dental, vision, and hearing?
Original Medicare (Parts A and B) does not cover routine dental, vision exams, eyeglasses, or hearing aids. Some Medicare Advantage plans include these benefits. Coverage and benefit limits vary widely by plan — check the specific plan's Evidence of Coverage document for details.
What is Medigap and do I need it?
Medigap (Medicare Supplement) insurance fills gaps in Original Medicare — primarily the 20% Part B coinsurance and the Part A deductible. Original Medicare has no out-of-pocket maximum, so a serious illness can result in unlimited cost-sharing. Medigap Plan G covers virtually all cost-sharing after the $283 Part B deductible, at a typical cost of $100–$180/month for a 65-year-old. For heavy utilizers, Medigap typically pays for itself.
